Donation system is up and open. We do not take cash. If you want to donate money you can send a check or money order and cashier check to P.O. Box. Credit card Visa and debit card visa accepted.

Instruction for donation form
1.) click on donation form to open the donation form
2.) open the pdf using the download button ( best used if you have the free adobe acrobat reader)
- having the adobe acrobat reader makes filling out the form simpler. This will also help with keeping you from printing out the form.
3.) when the document is opened you can begin to fill out the form.
4.) Fill in the date, Use your full name, address, and phone number (everything is confidentially and will not be used for any other purposes other than documenting who donated what)
5.) mark the amount of which you are planning to donate
6.) select if you are donating one time or if you plan to do it monthly
7.) enter the amount you are donating again on both lines that ask for re-entry of amount and on monthly or one time
8.) print your name, date, and initial your name
9.) after you have finished save the form and email it info@hmongchaofastate.com
*When emailing please put in the subject line that it is the donation form
*there will be an email sent back to you to confirm that your donation form was received.
*if there is no confirmation reply within 48 hours, contact us before emailing it again.

VOLUNTEER
The form to be able to volunteer is below. after filing it out email it to info@hmongchaofastate.com with the subject line indicating that it is about volunteering. Thank you in advanced!
